Parents Journal
Start a binder include everthing from initial Doctor visits, test, evaluations,
habits, likes, dislikes, contacts for services, future plans, finicial supports,
account numbers, letters of instructions, medical history, family members and contacts,
and Pictures of them. Always keep updated information in side.
If something were to happen to you the 'caregiver' would anybody else know what
services that are being provided to your loved one? Will they know where all
the important documents are? Will they know the likes, dislikes and the habits of
your loved one?
The Journal is to make the transition easier for your loved one and a new care giver.
Suggestions:
*Update on a regular basis (every two years)
*Give a copy to the person who is going to be advocating for your child
*include pictures of family members or important people in your child's life
